Requirements:
1. Megan's Law clearance for both Level I and II
Megan's Law Level I form / transfer form if you already have one at another PUSD School
TB testing - every 4 years. Free testing available through PUSD.

Request for Free Fingerprinting Service document approved by Mr. Hall, our BMMS principal.
2. Available to coach 1 hour a week or as needed from Sept. 18, 2011 to Feb. 4, 2012.
No special education required, most coaches are parent volunteers excited about offering this opportunity to our students, and interested in learning more about these subjects themselves.
Practice schedule times and locations are selected by each coach. BMMS classrooms can be reserved for free at the beginning of the season through the head coach and are available M-F 4pm to 9pm. We ask that practice times and locations be submitted for the calendar by Aug. 15, 2011.
3. Able to submit supplied attendance and citizenship records.
Download here. The head coach will need this completed record for every event to ensure a fair regional team selection for our students. Feel free to ask an assistant coach to help with some record-keeping.
4. Available on the Dec. 10, 2011 for BMMS SO Day and Feb. 4, 2012 for Regional Competiton.
Supplies:
Most supplies are already available, lab coats, goggles, basic calculators, etc.. There is a small budget available for each event for the entire season, approximately $75.
Coaches need to fill out a
Request for Reimbursement Form, sign it, and submit to the BMMS office. There is about a 3 week turnaround time and checks are automatically mailed to your home. If your seasonal budget exceeds this amount, approval from the head coach is necessary prior to purchase.
